Partnering with Patients in New Ways

The Institute of Medicine's definition of Self-Management Support encourages providers to use education and other supportive interventions in order to systematically increase patients' skills and confidence in managing their health problems. This suggests partnering with patients in new ways. 

 

For over a decade, the MacColl Institute has disseminated  the Chronic Care Model (CCM). The CCM identifies several elements that are essential to the delivery of excellent chronic illness care. MacColl researchers have advocated Self-Management Support (SMS) and the key contribution that support for patient self-management makes to chronic care. This site is indebted to their work and references the Chronic Care Model extensively.

Here are some things a patient and providers (in most cases a provider team) need to do in order to manage patient conditions, achieve better outcomes and ensure that their partnership is successful.

Many clinicians already do the things listed in the provider list. Assessment of Primary Care Resources and Supports for Chronic Disease Self-Management

 

This survey, developed by the Robert Wood Johnson Foundation Diabetes Initiative, offers primary care teams a tool for assessing and improving self-management support. The assessment tool and its recommendations are consistent with the Chronic Care Model and are formulated specifically to address self-management of diabetes and other chronic diseases.
